Pay Range: $29.00 $47.00/hour
Sign-On Bonus: $5,000

Registered Nurse Case Manager Opportunity:

• Collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
• Coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
• The Registered Nurse Case Manager will monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care

Registered Nurse Case Manager Qualifications:

• Must be a licensed Registered Nurse (RN) or Licensed Social Worker (LSW/LCSW)
• Must have previous experience as a Registered Nurse Case Manager or Case Manager
