REGISTERED NURSE

Signature HealthCARE seeks a compassionate Registered Nurse to deliver resident-centered clinical care in our long-term and post-acute facility. The RN will perform assessments, administer medications and treatments, coordinate care plans, and collaborate with an interdisciplinary team to promote dignity, safety, and quality outcomes. Responsibilities

• Perform comprehensive resident assessments and ongoing monitoring
• Administer medications and treatments per physician orders and facility policy
• Develop, implement, and update individualized care plans
• Collaborate with interdisciplinary team to coordinate resident care and discharge planning
• Document assessments, interventions, and resident responses accurately and timely in the medical record
• Supervise, mentor, and support CNAs and other support staff
• Educate residents and families on conditions, medications, and care plans
• Respond to changes in resident condition and emergencies using sound clinical judgment
• Participate in quality improvement, infection control, and safety initiatives
• Promote a resident-centered, compassionate, and dignified care environment
