Job Description

Signature HealthCARE seeks a compassionate Registered Nurse to deliver resident-centered care in our long-term and post-acute facility. The RN will assess residents, administer medications and treatments, coordinate care plans, and document clinical status. You will collaborate closely with an interdisciplinary team to support rehabilitation goals and quality-of-life outcomes. Responsibilities

  • Assess, plan, implement, and evaluate resident care plans in a long-term and post-acute setting
  • Administer medications, treatments, and procedures per physician orders and facility policies
  • Monitor, document, and report changes in resident condition using electronic health records
  • Collaborate with an interdisciplinary team (therapy, social services, dietary, physicians) to support resident outcomes
  • Educate residents and families on diagnoses, medications, and care plans
  • Supervise, mentor, and support CNAs and other support staff to ensure high-quality care
  • Ensure compliance with facility policies, state and federal regulations, and infection control standards
  • Participate in care conferences, shift reports, and quality improvement initiatives
  • Respond promptly to emergencies and perform BLS/ACLS as appropriate
