Job Description

POSITION SUMMARY:

The Community-Based Clinician (CBC) serves as a clinician, trainer, and relationship builder. The CBC will work closely with a team of peers and other clinicians to perform community-based outreach and trainings, with the goal of connecting people to START or other health-related services, as well as building general awareness about behavioral health. The CBC role requires administrative duties, including recipient-related and interdisciplinary team documentation. The CBC will foster a practice environment with a health equity focus that is patient-centered, trauma-informed and applies evidence-based treatment and harm reduction interventions and principles to support people with substance use and/or mental health disorders.
This is a grant funded position.

 

ESSENTIAL FUNCTIONS: The Community Based Clinician’s duties will be shared across the following areas:

Street-Based Outreach and Engagement:

  • Conduct community-based field work focusing on people who use drugs, people with serious mental illness, and/or experiencing homelessness.

  • Engage community members through street outreach, with the goal of providing psychoeducation on stigma, mental health, substance use, addiction, harm reduction and recovery.

  • Perform mental health and substance use assessments, including risk and safety determination and planning.

  • Link engaged persons to START and/or other health-related support and services, and conduct timely follow up.

  • Initiate appropriate START field-based intakes and enrollments.

  • Document engagements with community members.

Community-Based Training and Capacity Building

  • Engage community members at designated health promotion events to provide psychoeducation on mental health, substance use, stigma, harm reduction, addiction, and recovery.

  • Refer and link community members to START and other health-related support and services.

  • Serve as a lead facilitator of community-based harm reduction trainings, including Naloxone delivery and overdose prevention education and evidence-based harm reduction interventions.

  • Engage and build relationships with local community-based organizations.

  • Provide psychoeducation to START staff (e.g., counselors, peers, etc.) focused on community-based care and complex care management for high-risk persons.

  • Other duties as assigned.

Professional Development

  • Maintain clinical knowledge and skills based upon current social work and health care practices.

  • Maintain membership in professional social work organizations.

  • Maintain the standards and educational requirements specific to the social work role and licensure.

  • Maintain and update clinical knowledge and skills based on current health care practice/research

 

REQUIRED QUALIFICATIONS & COMPETENCIES:

  • Masters or doctoral degree in social work, mental health counseling, psychology, or other clinical field;

  • Licensed Master Social Work (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or other clinical licensure

  • At least one (1) year of experience with community-based outreach, engagement, or care

  • At least three (3) years of experience conducting standardized behavioral health assessments, risk assessments, and providing treatment recommendations and longitudinal follow-up.

PREFERRED QUALIFICATIONS

  • Bi/Multi-lingual

 

ADDITIONAL REQUIREMENTS

  • Able to travel throughout the communities we support, primarily, NYC and Brooklyn.

  • Able to work outside the traditional hours of 9:00 am to 5:00 pm.
