An Outreach Specialists primary responsibility is to find member populations that are difficult to locate and get them the preventative health services to which they are entitled.
Essential Duties and Responsibilities:
Provide progressive and meaningful face to face, telephonic, and written outreach and orientation services for each referred member to facilitate Health Home screening.
Meet established goals and deadlines to include addressing identified gaps.
Obtain applicable consent and enrollment forms when necessary.
Schedule appointments with designated clinicians for each contacted member.
Follow-up with each member to remind them of upcoming appointments and confirm completed appointments.
Reschedule appointments if not completed.
Maintain accurate case records and contact notes documenting all efforts within designated time of attempted/ completed service delivery.
Perform other duties as assigned.
High School Diploma or equivalent required; some college preferred.
Minimum 2 years of customer service and/or sales service.
Computer proficiency and software skills required; CRM experience preferred.
Access to reliable transportation.
Strong organizational and communication skills.
Time Management: You will need to be able to task yourself and members who are participating in the program.
Self-Motivation: You will need to possess the ability to work independently to manage schedules.
Interpersonal Skills: You will be working with other staff members, but you will also interact with the public. Must possess outgoing, hard-working attitude and an ability to positively influence others.
