Heaven’s Hands Community Services (HHCS) is a not- for- profit agency established October 06, 2006. We continue to lead our mission to be the preferred provider of services and supports to individuals with intellectual and developmental disabilities. We are dedicated to providing an array of high quality services and supports to the families, individuals and communities we serve.

Heaven’s Hands Community Services is seeking a Medicaid Service Coordinator (MSC). The Medicaid Service Coordinator will be responsible for assisting individuals to obtain the services and supports that they need and want in order to maximize their independence, individuality, inclusion in the community, and productivity.
Duties and Responsibilities:

•Provide high quality, person centered services – assisting individuals to maximize their independence, individuality, inclusion in the community, and productivity.
•Assist individuals to obtain the services and supports that they need and want by informing individuals of the various services available to them, referring them to said services, and monitor consumer satisfaction with services rendered.
•Maintain and obtain benefits and entitlements.
•Assist the individual with maintaining a healthy lifestyle.
•Develop, maintain, and implement and Individualized Service Plan annually with individual and family input. Review the plan as needed, at least every six months.
•Coordinate and facilitate the ISP meeting with individuals, families, and other service providers. Distribute copies of ISP to all appropriate parties.
•Document all meetings, referrals, issues, and concerns within the monthly progress note.
•Submit all documentation and paperwork in a timely fashion as delegated by supervisor.
•Maintain individual records as per OPWDD requirements.
•Conduct monthly face-to-face meetings of individuals within case load. Ensuring that every three months face-to-face meetings are held in the individual’s home. Document all face-to-face meeting with the contact verification sheets.
•Provides on-going service coordination to a caseload of 40 units. .
•Attend conferences, in-services, trainings, and informational meetings as assigned by supervisor and mandated by OPWDD to enhance and update job performance skills.
•Provides advocacy and support services to consumers and their families as needed.
•Remain on call 24 hours a day in the event of an emergency.
•Other duties as assigned by your supervisor.

Qualifications:
•Must have an Associates or Bachelor’s degree in Human Service or Psychology field
•Must have a minimum of 1 year service coordination experience working with developmental disabilities individuals.
•Must have a working knowledge of the current regulations, policies, and protocols governing Medicaid Service.
•Bilingual (Spanish-English speaking) preferred
•Is available to work outside of regular scheduled work hours as needed. .
•Able to research and link client with appropriate services.
